Thank you for investing and empowering the next generation with JA! Please see below for pre- and post-program forms and evaluations.

Before volunteering, please plan to take 5 minutes and complete the Volunteer Information form; if you will be appearing on camera, please also complete the Media Release form.

After your JA program experience, we would be very grateful if you would help us improve by completing either the Standard or JA in a Day evaluation. If you volunteered in multiple classrooms this year, please consider completing one survey for each class taught. Your feedback is invaluable! Thank you!
